Laurel M. Tucker, MD
Dr. Tucker received her medical degree from Indiana University School of Medicine in 1993. She began practicing Family Medicine in Indiana in 1996. Dr. Tucker moved to Texas and began practicing here in 1998. She is board certified with the American Academy of Family Physicians and an active member of the Texas Medical Association and Harris County Medical Society. Dr. Tucker treats all ages, from pediatric to geriatric patients. Her aim is to provide compassionate and comprehensive care to all her patients, promoting the benefits of preventive care and healthy living throughout life. Dr. Tucker has six wonderful children, a supportive husband and enjoys being active with her family.

Anthony J. Popek, MD
Dr. Popek received his medical degree from the University of Texas Medical School at Houston in 1986. He received his board certification in 1989 from The American Board of Family Medicine and began his practice shortly after. He is a member of the Harris County Medical Society serving as the Health Insurance Medical Liaison. His purpose is to provide quality care to all his patients. Dr. Popek has one teenage daughter and a lovely wife. He enjoys working around his house and gardening.

Abigale Darnell, NP-C
Abigale joined Town and Country Family Physicians in the Fall of 2024. She received her Doctorate of Nursing Practice degree as a Family Nurse Practitioner from the University of Texas Health Science Center at Houston. Prior to this, she worked as a Registered Nurse in the Emergency Department for 7 years at various hospitals in the Greater Houston area. She enjoys taking care of patients across the lifespan promoting disease prevention and promoting optimal living but is especially passionate about Women's Health issues. During her free time, she enjoys spending time with her family and friends, reading, and traveling.

Lubna Momin, NP-C
Lubna joined Town and Country Family Physicians in the Spring of 2017. She received her Master of Science in Nursing degree as a Family Nurse Practitioner from the University of Texas Health Science Center at Houston. Prior to this, she worked as a Registered Nurse at the M.D. Anderson Cancer Center for over 3 years. Lubna speaks Hindi and Gujarati. She enjoys taking care of all patients across the lifespan and strongly believes in health promotion and disease prevention. During her free time, she enjoys spending time with her family and friends, watching movies, volunteering in the community, and spending time outdoors.

Hannah Dai, NP-C
Hannah is a Family Nurse Practitioner with a Doctor of Nursing Practice (DNP) degree from the University of Texas Health Science Center at Houston and board certification as a Family Nurse Practitioner (FNP-BC). With a deep commitment to providing compassionate, patient-centered care, Hannah focuses on empowering individuals and families to take an active role in their health and wellness. Hannah has developed extensive clinical experience across a variety of healthcare settings. This diverse background allows her to deliver comprehensive care to patients of all ages, from managing acute conditions to providing long-term wellness strategies. Hannah takes pride in tailoring care plans to meet each patient’s unique needs, fostering a collaborative approach that promotes overall health and well-being. In addition to clinical practice, Hannah is passionate about ongoing professional development and staying at the forefront of healthcare innovations to provide the best possible care. Outside of the office, Hannah enjoys reading and spending time with her friends and family.
