We at Town & Country Family Physicians are excited to announce that we are upgrading to a new patient portal called MyHealthRecord.com. It is your health record and connection to our practice from any device, like tablets, smartphones and desktop computers. Whether you use an iPhone, Android tablet or full desktop computer, you will have full access to all features on all devices. We will send you an email inviting you to MyHealthRecord.com. The invitation will include instructions, which is a quick, three-step process.

Requirements to Qualify for Invite.
• First name
• Last name
• Date of birth
• Zip code (5 digit only)
• Email address
Patient information must match the same as we have in our system in order to verify identity.

Using MyHealtRecord.com you can connect with our office through a convenient, safe, and secure environment to:

• Request appointments
• Request medication renewals
• Receive lab results
• Send and receive secure messages to and from our office
• View the medical chart with health record details
• Receive and review documents
• Update account information
• View statements online- requires patient to send request
• Make payments online

Using myhealthrecord.com patient portal can eliminate the phone tag that patients often encounter when calling the office by sending our office a secure message.
